Charles B. Wang Community Health Center is Moving to MyChart Patient Portal!
On, Tuesday, September 16, 2025, the Charles B. Wang Community Health Center will be moving to a new and secure patient portal powered by MyChart. This new portal will provide you with 24/7 access to your health information, tracking of your care and progress, requests for appointments or medication refills, and much more!
Effective September 16, 2025, your old login information will no longer be active from the ezAccess patient portal. All medical records will be transferred except for your inbox messages. You can still download any existing messages, if needed, up until Friday, September 12.
You will receive information on how to sign up to MyChart via email and/or text after September 16, 2025, or when you arrive for your next appointment.
*The patient portal is not for urgent requests. For urgent requests, please call your healthcare provider’s office. If you are experiencing a true medical emergency, please call 911 or go to the nearest emergency room.
